Volunteer Application Forms
All AYSO Volunteers who come into contact with players are required to fill out and turn in a volunteer application annually. This enables us to complete the necessary background checks.

Once you are logged in, click the large "AYSO Volunteers" button to get started.
After you fill it out, you will need to print it and turn it in to a qualified board member who will need to see photo identification for verification purposes. This process must be completed before being accepted as a coach, assistant coach, referee, or any position in which contact with players occurs.

Registration Instructions (in-person or by mail)
Fall 2008
- Logon to www.eAYSO.org and complete the on-line registration process. If you’ve used the system before, you can login in w/ your username & password and simply update your child’s form. If you are new to Eugene AYSO, you will need to create a username & password and complete the entire registration form. It typically takes 15 minutes for first time on-line registration. Reminder: you will need to press “submit” or “next” at the bottom of every page of questions to progress through the form. The system should generate a one page registration form with a one page liability waiver attached. When you’ve completed the process, you will have a total of 4 pages to bring with you to registration. No computer? Come to one of our Open Registrations, dates below, and you can complete the paperwork.
- Player or coach requests: If you have a preference for your child’s coach or if you child would like to play with his or her friend, please write the first & last name of this individual on the top of your form. You can also use the “sibling” box if you are filling out the form on-line. Remember, this is a REQUEST, not a guarantee.
- Print: 2 copies (one goes to the coach & one stays in the office to track team placement & for insurance purposes). We do need 2 copies – if you’d like your own copy, print a third form.
- Sign & date the bottom of the form. Please use the date that you are actually submitting the form.
- Payment: Enclose a personal, business or cashier’s check made payable to AYSO Region 53. Cash payments can be taken at open registration dates (below).

Get it to us! We don’t send bills out or call & remind folks to pay. You are responsible for this step. Registrations must be received at the AYSO p.o. box no later than Saturday, July 26, 2008. It is possible, but not guaranteed, that late registration will take place at a time & location to be announced. Your best bet for ensuring your child a team is to complete registration by July 26th for inclusion in 1st round team formation. Coach & player requests will not be honored for late registrants.

- Open Registration: Registration takes less than 5 minutes if you bring the pre-printed forms & your check or cash ready to go to one of the following dates – registration will be at the AYSO Office/Andersen Woodworks, 4697 Isabelle Street, Eugene. Registration forms will not be taken at other times.
July 12th from 10 a.m. to 1 p.m.
July 19th from 10 a.m. to 1 p.m.
July 22nd from 6 to 8:30 p.m.
- Can you help make your child’s season happen? We hope so! All of these positions need to be filled to run a great season: trained & background checked coaches & referees, field lining crews, board of directors help, registration help, Equipment Exchange sale help, equipment sorting & packing, game field set-up on Saturday mornings at Churchill.
- What now? Check on-line for additional registration information and for season start dates. The web-site, www.eugeneayso.org, is a great source for continued information on season dates and volunteer opportunities and photos. Check it out!
- Here’s why AYSO is different: Eugene AYSO is a co-ed, all volunteer, player-focused soccer organization. We encourage our parents & players to have fun playing soccer in a family-friendly environment. If you experience something on the practice or game field that does not meet these standards, please share your concerns with your coach, the referee (after the game), other parents, and AYSO board members. You can reach all AYSO board members through our message line at 484-1149. In addition, AYSO board meetings, which occur the third Wednesday of every month, are open to AYSO members. We encourage & expect our parents to be part of the solution & to be fair to all players & volunteers and their families.
- The very last thing: Many of the fields we use belong to the 4J School District. The District has a ‘no dogs’ policy. In addition, as part of AYSO’s primary focus on safety for our kids, we also have a ‘no pets’ policy. We love dogs, just not at AYSO games or practices. Please allow our volunteers to keep working at the other important tasks and take personal responsibility for your pet by keeping it home.

REMEMBER:
NO PETS ARE ALLOWED ANYWHERE NEAR AN AYSO GAME OR PRACTICE
This is a longstanding rule, put in place to avoid a repeat of several instances that have occurred in the past -- including animal bites, scratches, game disruptions, messes on the field of play, and dogfights.
To be fair to everyone, we have a blanket policy: You will be asked to remove your pet if you bring it. Yes, even if it is a "nice" pet. Sorry for any inconvenience, but please leave them at home during practices and games.
